What TrumpRx actually is
TrumpRx is a federal direct-to-consumer prescription program announced in November 2025 and operational in early 2026. It's part of the Trump administration's Most Favored Nation (MFN) pricing initiative, which aims to align U.S. drug prices with the lower prices paid in other developed countries.
Practically, TrumpRx is a website and enrollment portal that lets U.S. residents order certain FDA-approved medications directly, at negotiated cash-pay prices, without going through traditional insurance. For GLP-1 drugs, this represents one of the most significant pricing changes since the class launched.

Key numbers to remember: Zepbound and Wegovy drop from over $1,000 to about $350 per month. That's roughly a 65-74% reduction from list. Mounjaro is slightly lower at $325.
How the Medicare $50 tier works
The White House fact sheet outlined a $50/month tier specifically for Medicare beneficiaries. To qualify:
- Medicare Part D enrollment is required.
- The drug must be prescribed for its FDA-approved indication. For example, Zepbound and Wegovy are approved for obesity (BMI ≥30, or ≥27 with a weight-related comorbidity). Mounjaro and Ozempic are approved for type 2 diabetes.
- A valid U.S. prescription from a licensed prescriber is required.
Prior to TrumpRx, Medicare Part D generally did not cover obesity GLP-1s for weight loss alone. The $50 tier is a structural change to that coverage pattern for MFN-participating drugs.
Pill vs. shot — how future oral GLP-1s fit in
The White House fact sheet also outlined pricing for oral GLP-1s that don't yet exist in the U.S. market:
"In the event that the FDA later approves the Wegovy pill, or certain similar 'GLP-1' drugs in each company's pipeline intended to be taken orally rather than as a shot, the initial dose of those drugs will be priced at $150 per month through TrumpRx." — White House Fact Sheet, November 2025

How TrumpRx compares to LillyDirect and NovoCare
LillyDirect
LillyDirect is Lilly's own direct-to-consumer channel for Zepbound and Mounjaro. Cash-pay pricing runs approximately $349-$499 per month depending on dose and shipping frequency. For most patients, TrumpRx and LillyDirect end up at very similar prices for Lilly products.
NovoCare Pharmacy
NovoCare Pharmacy is Novo Nordisk's direct channel for Wegovy. Cash pricing has historically been higher than TrumpRx — around $499-$650 per month depending on dose. TrumpRx meaningfully undercuts NovoCare for cash-pay patients.
Manufacturer savings cards (if commercially insured)
Zepbound's savings card can bring commercially-insured patients to as low as $25/month with a maximum benefit period. Wegovy has a similar card program. For patients with good commercial coverage that includes GLP-1s, savings cards often produce a lower net cost than TrumpRx.
Enrollment — step by step
- Get a prescription. You need a valid U.S. prescription from a licensed prescriber. Telehealth platforms like Henry Meds, Ro, or your primary care physician can prescribe if medically appropriate.
- Verify eligibility on TrumpRx.gov. Enter your prescription and insurance information (if you have any). The system checks whether you qualify for the Medicare $50 tier or standard cash pricing.
- Choose fulfillment. TrumpRx uses partnered pharmacies for fulfillment. Delivery typically arrives within 5-7 business days.
- Refill setup. Prescriptions can be set to auto-refill monthly. You can pause or cancel at any time.
Who saves the most under TrumpRx
- Uninsured patients paying cash — largest absolute savings vs. list price.
- Medicare Part D patients with an FDA-approved indication — the $50/month tier is transformative for previously-uncovered obesity treatment.
- Commercially-insured patients whose plans don't cover GLP-1s for weight loss — many plans still exclude obesity GLP-1s.
Who might do better elsewhere
- Commercially-insured patients with GLP-1 coverage — manufacturer savings cards may drop net cost below TrumpRx.
- Patients whose insurance already covers Wegovy or Zepbound at $25-$50 copay — no need to switch.
- Patients seeking compounded semaglutide/tirzepatide — TrumpRx only sells FDA-approved branded products; compounded versions (where legally available) may be cheaper but come with their own risks and current legal restrictions.
What about retatrutide?
Retatrutide is not included in TrumpRx because it is not FDA-approved. Lilly has stated it plans to file its Biologics License Application in Q1 2027, with a possible decision in late 2027 or 2028.
Any retatrutide pricing under TrumpRx would only be established after FDA approval and separate negotiation. Anyone claiming to sell "retatrutide" via TrumpRx today is not legitimate — as of August 2026, Lilly is actively suing black-market retatrutide sellers.
